Parent Waiver (Please Read Carefully): By checking this box I agree with the following statement - I, parent/guardian, hereby waive any and/or all rights, claims for damage arising from injury received while my child is playing games and/or practicing, walking, or being transported to games or other associated activities. I release indemnity and hold harmless the Boys Baseball Association (BBA) Grove Park, its board of directors, organizers, coaches, sponsors, managers, or any other appointed supervisors for any injury incidental to the activities or transportation to and from these activities. I acknowledge that participation in baseball may expose my child to certain risks associated with sports competition. I give full consent and permission for my child to be transported by ambulance or aid car to an emergency center for immediate treatment and procedures, if deemed necessary. I also give permission for the BBA to photograph and use my child's picture for promotional purposes.

The League will provide cap, jersey, belt and socks for registered players age 6 and above. For T-Ball players, age 3-5, jersey and cap.
